- This service is not available for patients under the age of 16.
- You must have your own, unique email address, so for example a couple cannot use the same email address, nor can you use your login to book appointments for anyone other than you.
- By giving us your mobile telephone number, we may use this to contact you to remind you about appointments you have booked.
You can extend your online account access to include your medical record summary. However, if you wish to access this, we will need to verify your identity. You will need to complete the form at this link, print it out and come in with either a passport or photo driving licence or a utility bill such as a council tax statement. Please note – THESE ARE THE ONLY FORMS OF IDENTITY WE ARE ALLOWED TO TAKE. This extended access option is not available to patients under the age of 18.
